FAQ

Certificates FAQs

Where do I get a medical certificate?

We issue them at the reception desk. Visit during clinic hours with your ID to apply.

Can a family member receive a medical certificate on my behalf?

No. Under Article 17 of the Medical Service Act, if the patient is conscious, a medical certificate cannot be issued to a proxy, so only the patient can receive it.

Can a proxy receive copies of my medical records?

Yes. Please bring the applicant’s ID, a copy of the patient’s ID, and the consent form and letter of authorization bearing the patient’s handwritten signature.

Where can I get the consent form and letter of authorization?

They are available at the reception desk and must be completed using the statutory forms, Attached Forms No. 9-2 and 9-3.

Can I use a seal/stamp?

No. Consent forms and letters of authorization must bear a handwritten signature; stamps or fingerprints are not accepted.

How do I receive documents for a child under age 19?

Bring the ID of the parent with parental authority and a document verifying parental authority (e.g., a family relations certificate).

How much is the issuance fee?

Certificate fees follow the standards posted in the clinic. They vary by document type and number of pages, so please check with the reception desk or our main line at +82-2-555-1369.

I don’t know what documents my insurer needs.

Each insurer requires different documents. Please confirm with your insurer first and then apply, so you don’t have to make a second trip.

Which certificates can I receive?

We issue general medical certificates (Korean and English), Military Manpower Administration medical certificates, injury medical certificates, medical opinions (Korean and English), medical opinions for insurance claims, certificates of treatment and of surgery, certificates of outpatient visits, copies of medical records, and CD copies of test images. Depending on the document, some can be received only by the patient, while others may also be received by an authorized proxy.

Do I need an appointment to receive them?

No appointment is necessary. Visit the front desk during clinic hours to apply.

How long does issuance take?

It depends on the document type. The front desk will inform you when you apply.

Can I receive a document if I forget my ID?

Identity verification is required, so issuance is difficult without an ID. Even when a proxy applies, the applicant’s ID and a copy of the patient’s ID are required.

Can I have a medical certificate reissued?

You can reapply whenever needed. The required items are the same as for the initial issuance, and fees follow the standards posted in the clinic.

What information is included in a copy of the medical records?

It includes records generated during care, such as the initial visit record and test record. If you need imaging, you can also receive a CD copy.

If my spouse or child is picking up a copy of my medical records on my behalf, what is required?

The spouse, lineal ascendants and descendants, and the spouse’s lineal ascendants should bring the applicant’s ID; a copy of the patient’s ID (if age 17 or older); a document verifying the family relationship; and a consent form bearing the patient’s handwritten signature (if age 14 or older). A proxy who is not a family member, such as a brother or sister or an insurance company employee, also needs a letter of authorization.
